A supervision log tracks every supervision meeting systematically. Use this template to create a complete, accurate record of your supervision hours for licensing board verification.
Why Accurate Hour Logging Matters
Licensing boards require documented proof of supervision hours as part of the licensure application. Incomplete, inaccurate, or poorly organized logs can delay your application, trigger additional review, or even result in denied hours.
A well-maintained supervision log removes uncertainty. When it's time to submit your licensure application, you'll have a clear, verified record showing exactly how many hours you completed, when, with whom, and in what format.
What to Track in Your Supervision Log
Each entry in your supervision log should capture the following information:
Supervision Meeting Information
- Date: The exact date of the supervision meeting
- Start and end time: Record actual times, not just duration
- Duration: Total hours and minutes (calculated from start/end times)
- Format: Individual supervision or group supervision
- Modality: In-person, telehealth (video), or phone
Supervisor Information
- Supervisor name: Full name and credentials
- Supervisor license number: For verification purposes
- Supervisor signature: Physical or digital signature confirming the supervision meeting occurred
Supervision Content
- Topics covered: Brief description of what was discussed
- Cases reviewed: Number or identifiers of cases discussed
- Category: Direct clinical hours, indirect hours, or administrative (if your state differentiates)
Running Totals
- Cumulative individual hours: Running total of individual supervision hours
- Cumulative group hours: Running total of group supervision hours
- Total supervision hours: Combined total
- Hours remaining: Based on your state's total requirement

Best Practices for Supervision Logging
- Log every supervision meeting immediately. Don't wait until the end of the month. Log the supervision meeting the same day it occurs.
- Get signatures promptly. Obtain your supervisor's signature at the end of each supervision meeting or within the same week.
- Track by category. Many states distinguish between individual and group supervision hours, and between direct and indirect clinical hours. Track these separately from the start.
- Maintain running totals. Keep a running count so you always know how many hours remain. This prevents surprises when you're ready to apply.
- Back up your records. Keep copies in multiple secure locations. Losing your supervision log could mean losing hours of documented work.
- Verify requirements early. Check your state board's specific log format requirements. Some boards provide official forms; others accept custom logs that meet their criteria.
