Best Practices for Supervision Notes
- Write notes promptly. Complete supervision notes within 24-48 hours while details are fresh.
- Be specific but concise. Include enough detail to demonstrate the substance of supervision without writing a narrative. Focus on clinical content and professional development.
- Use consistent formatting. A standard structure makes it easy to complete notes efficiently and ensures nothing is missed.
- Protect client confidentiality. Use initials or case numbers rather than full client names.
- Store securely. Supervision notes are clinical documents and should be stored in a secure, HIPAA-compliant manner.
- Retain appropriately. Follow your state's record retention requirements, typically 7 years minimum, longer in some jurisdictions.
Common Mistakes to Avoid
- Writing vague notes like "discussed cases" without specifics
