Supervision Note Template

Supervision notes document what happened in each supervision meeting. Use this template to create consistent, thorough records that support both clinical development and licensure documentation.

Why Supervision Notes Matter

Supervision notes serve two critical purposes: they support the supervisee's clinical growth by creating a record of feedback, goals, and progress. They also provide the documentation that licensing boards may request as evidence of adequate supervision.

Consistent documentation also protects both parties if questions arise about the supervision that was provided. Notes demonstrate that the supervisor was engaged, providing feedback, addressing ethical concerns, and supporting professional development.

What to Include in Each Supervision Note

Supervision Meeting Details

  • Date and time of the supervision meeting
  • Duration (start and end time)
  • Format: individual or group supervision
  • Modality: in-person or telehealth
  • Names of all participants (for group supervision)

Cases Reviewed

  • Client identifiers (initials or case numbers, never full names in supervision notes)
  • Presenting issues discussed
  • Clinical interventions reviewed
  • Diagnostic impressions or updates
  • Treatment plan progress or modifications

Clinical Skills and Competencies

  • Specific skills practiced or discussed
  • Theoretical approaches applied
  • Areas of strength identified
  • Areas for growth or improvement

Ethical and Legal Issues

  • Any ethical dilemmas discussed
  • Boundary concerns
  • Confidentiality considerations
  • Mandated reporting situations reviewed
  • Risk assessment discussions

Feedback and Directives

  • Specific feedback provided by the supervisor
  • Recommended readings, trainings, or resources
  • Clinical directives or instructions
  • Any plan modifications requested

Action Items and Follow-Up

  • Tasks assigned to the supervisee before the next supervision meeting
  • Cases to follow up on
  • Topics to carry over to the next meeting
  • Goals for the coming week

Signatures

  • Supervisor signature and date
  • Supervisee signature and date (when required by your state)

Best Practices for Supervision Notes

  • Write notes promptly. Complete supervision notes within 24-48 hours while details are fresh.
  • Be specific but concise. Include enough detail to demonstrate the substance of supervision without writing a narrative. Focus on clinical content and professional development.
  • Use consistent formatting. A standard structure makes it easy to complete notes efficiently and ensures nothing is missed.
  • Protect client confidentiality. Use initials or case numbers rather than full client names.
  • Store securely. Supervision notes are clinical documents and should be stored in a secure, HIPAA-compliant manner.
  • Retain appropriately. Follow your state's record retention requirements, typically 7 years minimum, longer in some jurisdictions.

Common Mistakes to Avoid

  • Writing vague notes like "discussed cases" without specifics
  • Skipping supervision meetings or batching notes weeks later
  • Including full client names or other identifying information
  • Not getting signatures on notes when required
  • Storing notes in unsecured locations (email, personal drives)
